1) The Health Exams form has been updated to link the revised oral health assessment form and oral health assessment waiver form that the CDPH published in July 2022. Only the English versions have been updated. New Spanish versions have not been published.

2) The Health Exams form now includes variable links which can be used to link custom forms (such as those specific to your school district or local Dept. of Health).

3) The Most Recent School form has been updated to hide the previous school questions if not applicable.

2022-23

1) The Student Housing Questionnaire form has been updated to match the CDE revision dated November 2021 (modified per Assembly Bill (“AB”) 27 and Senate Bill (“SB”) 400).
Our form now includes a variable you can edit to add contact info for your LEA’s Homeless Liaison.

2) The Medical Information now includes a variable link which can be used to link a custom Authorization for Administration of Medication form.
